DCM
Healthcare providers are becoming increasingly dependent on data in electronic health records. This means that when data are shared it is extremely important for the recipient to be able to understand it fully and place it in the right context. Only then, the information can be used safely for the continuity of care for the patient. Detailed Clinical Models (DCM) provide the required unambiguity. Results4Care is happy to support you in the development of DCMs.
We completed DCM for Health Base for the knowledge in their general practitioner records.
The Onze Lieve Vrouwe Gasthuis Hospital in Amsterdam developed about 25 DCMs which form the basis of their current nursing record system.
The Perinatal Registration Netherlands (PRN) uses the DCMs to support midwifes and gynaecologists to include the required data in electronic health records.
For a personal health record, DCMs will be applied in the upcoming months.
In Zwolle the project of Isala and Icare to exchange nursing oncology data delivered its final product, the HL7 v3 care record populated with DCM based data in December 2013.
